The mission of Finnz Fundz is to provide financial assistance related to basic needs expenses. These include housing, utilities (including phone), transportation, and food. At this time, we are unable to consider requests for other types of assistance, including expenses related to medical bills, equipment, or prescription medication.
To request assistance from Finnz Fundz:

- In addition to this form, confirmation of Cystic Fibrosis diagnosis is required in order to process your request.
This can be a letter from a care provider (social worker, doctor, nurse), or a picture/copy of official paperwork (such as a hospital discharge) that indicates CF diagnosis.
The confirmation documentation can be emailed to firstname.lastname@example.org, or sent via text to (401) 527-1773.
Please be mindful of your own privacy when sending this information--personal information, such as social security numbers, should be blacked out for your protection. Medical information/documentation is not retained on file by Finnz Fundz; it is used for confirmation of diagnosis only, then will be deleted.
- Finnz Fundz is able to consider future requests for assistance no sooner than one year after a previous request was received. Future requests made after this date will be considered in the order they are received. The fulfillment of any future assistance requests is dependent on the funds available at the time of the request. Priority consideration is given to first-time requests.
Please note: Finnz Fundz does not provide assistance via fundraising websites such as gofundme.com. These websites take a percentage of funds donated as a fee, which can be upwards of 20% of the amount donated. We want the most money possible to go directly to those affected by CF.
